a lot of my pages are missing missing hreflang, any ideas on how to implement this across the site.

Does the issue occur when using a Twenty series WP theme?

Could it be because I am using a child’s theme

I don’t believe so unless you’ve added something in the child theme that caused it.
You can simply activate the parent theme to test.
